			<content:encoded><![CDATA[<p>I find myself running behind in the office and sometimes doing what I hate the most&#8230; keeping someone waiting for their <a href="http://www.atcosmetics.com" title="Cosmetic Surgery">cosmetic surgery</a> consultation. I am on your side here. I don’t like waiting in the doctor’s office either.</p>
<p>So first and foremost if you are reading this and I have kept you waiting, or will keep you waiting, <strong>I offer you my sincere apologies</strong>. There is more to this blog however. I thought you might find it interesting to know how and why we can get backed up.</p>
<p><b>&#8212;&#8211;</b>The cascading arrival time: While we have a good scheduling program all it takes is one late arrival to shift the schedule off kilter by 30 minutes or more.  Unfortunately that will happen on average about twice during an office day at anyone of my plastic surgery practices in the Washington DC, Virginia area.</p>
<p><b>&#8212;&#8211;</b>The exploding consult: for the average consultation we have a pretty good estimate the time it will take for me to evaluate, explain and go over treatments. However, it is extraordinarily common for a patient to book a consultation about one type of surgery but then also ask about two or three more. At 25- 30 minute per topic the math gets a little scary. Interested in knowing more about that topic, read my past blog <a href="http://www.atcosmetics.com/blog/what-goes-together-more-than-one-procedure-of-plastic-surgery/" title="What goes together">&#8220;What Goes Together- More Than One Procedure of Plastic Surgery&#8221;</a>.</p>
<p><b>&#8212;&#8211;</b>The mom /friend phenomena: I love it when a patient brings a friend or family member to the consultation another set of ears and eyes is comforting and helpful. It is extremely common though that the friend or family member becomes the second consult in that room at the same time.</p>
<p><b>&#8212;&#8211;</b>I am long winded: yes, it’s true.  I love to teach and educate my patients and I don’t want anyone to leave feeling they did not get the best and most informative experience they could. So if one topic becomes 3 or if mom or friend has a question I will always feel compelled to answer it in detail.</p>
<p>I hope this doesn’t scare you off.  A busy cosmetic <a href="http://www.atcosmetics.com" title="Plastic Surgery Maryland">plastic surgery</a> practice is a good thing.  </p>
<p>So a few tips: Morning consults are less prone to cascading delays, if you’re interested in many things, or so is your companion, just let us know ahead and we’ll leave more time. Bring a book just in case. 			<content:encoded><![CDATA[<p>When a patient comes in for a consult I never want that patient to feel as though I am trying to add additional <b>plastic surgery</b>, but it is my job is to point out where certain combinations are more powerful than just the one surgery alone. In plastic surgery there are certain procedures that must at least be considered together.</p>
<p>Some classic examples:</p>
<p><a href="/rhinoplasty/index.html">Rhinoplasty</a>:  Rhinoplasty really needs to be considered with the chin.  The reshaping of the nose has a dramatic effect on the profile.  If the chin is receding only half the battle is won. Chin implant may be an option.</p>
<p><a href="/html/face-lift.html">Eyelid surgery</a>:   The eye brow position really must be considered particularly when looking at the upper lids.  If the space between the brow and eyelid lashes is short and cramped eyelid surgery will have less of a result. Brow lift, or a combination of brow lift and eyelid surgery, may do a better job.</p>
<p><a href="/abdominoplasty/index.html">Tummy tuck</a> and flank liposuction:  Unless the skin is loose, many of my tummy tucks include frontal flank liposuction.  If the flank fullness goes around the backside, I will discuss liposuction of the posterior flank (love handles) as well.  The overall contour must be considered.</a></p>
<p><a href="/html/face-lift.html">Facelift</a> and eyes:  If the neckline looks tight and youthful but the eyes don’t match the face won’t balance.  Facelift is about the entire face and it all has to blend harmoniously.</p>
<p>This is not a complete list but I hope the concept comes across.  The parts of our body do not exist in complete isolation. My role as a board certified <a href="http://www.atcosmetics.com/">plastic surgeon</a> is to teach my patients where and how to look for the best balance.</p>

			<content:encoded><![CDATA[<p>A number of people have seen us over the past few years asking about the liquid or temporary <a href="http://www.atcosmetics.com/rhinoplasty/index.html">rhinoplasty</a>.  Popularized by a dermatologist in California, it is essentially a technique of using injectable fillers to alter the shape of the nose.</p>
<p>If a patient has a bump on top of the nose the bump can be camouflaged by adding filler just above it. In some cases mild asymmetry can be improved.  Eventually fillers will reabsorb and the procedure will have to be repeated.</p>
<p>I think it is a good option for people who are not ready to or healthy enough to consider <a href="http://www.atcosmetics.com/rhinoplasty/index.html">rhinoplasty</a>. It also allows a reversible change for people who don’t want to burn any bridges. Liquid <b>rhinoplasty</b> works  by enlarging specific areas, while standard rhinoplasty can alter the underlying bony structure giving permanent changes.</p>

<p><strong>Restylane</strong> is injectable facial filler made up of hyaluronic acid utilized to smooth lines and wrinkles on the face and to decreases moderate facial folds around the mouth and nose. But one not so marvelous thing during the season of gatherings, feasts and joy is the cold weather. During cold months, protecting your skin should be everyone&#8217;s top priority, old and young. With the <strong>DC, Maryland and Northern Virginia</strong> low freezing temperatures, snow and winds; scarves and other types of outwear accessories are the must haves to keep from frostbite happening on the face, ears, neck and mouth. But just like we must break out our winter wardrobe to prepare for wintertime blissfulness from November to March, we must also ready our skin for the cold to prevent it from damage, especially since the largest organ of the body is skin.</p>
<p>Cold weather can wreak havoc on the on your skin causing it to dehydrate. Dehydration can cause skin to feel dry and itchy, or crack and lips become chapped. On top of the damage our skin undergoes during the winter, wrinkles and fine lines can start to become more visible on dry skin causing people to look older then they really are. Then some feel they must undergo Botox, Restylane or another type of <a title="cosmetic surgery" href="http://www.atcosmetics.com" target="_self">cosmetic surgery</a> procedure to achieve their youthful appearence, which is not always the steps that need to be taken. Cold air is not the only factor for dry skin in cold months, but additionally time spent inside the well heated house can dry out the skin, decreasing the moisture in skin. <strong>To help keep your skin protected this winter,</strong> here are some hints:</p>
<p><strong>Add Greenery to Your Life </strong></p>
<p>During cold months in the DC, Maryland and Northern Virginia areas, redecorate the house, room or office space with some fresh new plants. House plants can promote hydration in your place since plants releases water into the air through the process of photosynthesis. Hydration in the air helps your skin maintain it&#8217;s moistness.</p>
<p><strong>Add a Layer over your Skin while Inside </strong></p>
<p>While in the house, it is essential to keep your delicate skin on your hands and feet in tact. To help keep from loss of moisture in your skin, wear gloves when you are washing the dishes. After washing your hands, add a layer of hand moisturizer. Rather than walking around barefoot in your house, wear socks made of cotton to keep in existing moisture.</p>

			<content:encoded><![CDATA[<p>We have noticed for years that Patients of ours with a history of migraines have seen subjective improvements in their migraine symptoms when using <a href="http://www.atcosmetics.com/html/generalprocedures.html#bo">Botox</a> even for cosmetic purposes.</p>
<p>As of last week the food and drug administration approved the use of Botox to treat patients with chronic migraines.  Chronic Migraines are defined as migraines more than 14 days each month each lasting more than four hours.</p>
<p>While we continue to perform <a href="http://www.atcosmetics.com/html/generalprocedures.html#bo">Botox</a> largely for cosmetic purposes  it is nice to know that our observations are founded and that this treatment can serve a dual role.</p>
<p>It is unclear at present whether insurance companies  will cover any portion of this treatment for  chronic migraine sufferers but don&#8217;t worrry we will be watching closely and let you know if it changes.</p>

			<content:encoded><![CDATA[<p>Every day I see ads in newspapers, magazines and circulars offering Botox® at insanely low prices. It leaves me wondering how patients really know that they’re getting good Botox treatment. Botox is reconstituted with sterile water. Add more water and you may stretch a bottle farther, but you also may see less of a result. Older Botox may not work as well as a freshly opened bottle. So, paying for Botox by the site can mean a better or worse result, depending on how generous or frugal your doctor is.</p>
<p>In my own practice we reconstitute the <a href="http://www.atcosmetics.com/html/generalprocedures.html#bo" target="_self">Botox®</a> strictly according to the manufacture’s guidelines. It is a concentrated solution that allows us to put smaller but stronger amounts in the areas of concern. This increases its effectiveness and minimizes its migration to areas we don’t want it to affect.</p>
<p>We have a steady stream of Botox patients, which allows us to use our supply of Botox quickly &#8211; meaning fresher and more effective Botox.</p>
<p>We also charge by the unit. What does that mean? Botox bottles contain one hundred units of botulinum toxin. Common areas treated include the area between the eyebrows (glabella), crow’s feet, and forehead. While the average full area will cost about $350, charging by the unit means our patients can buy more or less if they want. If a small area needs refreshing, we charge only for the units necessary and not the whole site. I think this is a more common sense approach.</p>
<p>Botox is not an exact science. We ask each of our patients to return in 10 days to evaluate the results of their treatment. By recording how satisfied they are and how they reacted to their last treatment, future treatment plans can be fine tuned. 			<content:encoded><![CDATA[<p>The names of procedures that are marketed today for facial rejuvenation can sound like alphabet soup and can be very confusing for patients.</p>
<p><a href="http://www.atcosmetics.com/html/face-lift.html">Facelifts</a> fall into one of two categories: Mini or Full. You may have heard of the ones with the cute names like S-lift, Thread lift, or Lifestyle lift. These are often marketed in the popular magazines, and are merely variations on the mini-facelift.</p>
<p>So which one do I recommend?&#8212;the one that fits my patient.</p>
<p>When a patient comes in to my office for a consultation I will listen to their goals and concerns, look at their hairline and hairstyle, and recommend a specific plan. I perform all of these procedures, but I will pick the specific components of each that will achieve the patient’s goals and try to minimize scars where possible.</p>
<p>I begin with the premise that no one wants the bigger facelift, but a patient with a turkey gobbler neck will need a more aggressive approach than the patient who comes in with a tad of neck laxity and a little joweling.  Some will benefit from an eyelid or brow lift, while others may not. Some don’t even need surgery at all, but may do better with laser, <a href="http://www.atcosmetics.com/html/generalprocedures.html#cp">chemical peels</a>, or fillers. Each patient is different, and each will need his or her own subtle surgical variations.</p>
